Efficacy of Thoracic Epidural Anaesthesia for Laparoscopic Cholecystectomy

Abstract 

Background and Aims: Laparoscopic cholecystectomy is performed to minimize the postoperative morbidity and early return to work. The thoracic epidural anaesthesia provides selective blockade at the surgical site and also diminishes the requirements of opioids and local anaesthetics. The present study was planned to evaluate efficacy of thoracic epidural anaesthesia for laparoscopic cholecystectomy, so that it can be later used as anesthetic technique in patients when general anaesthesia is not feasible. Methods: The study included total 30 patients of (age 20- 60 years) either sex, ASA grade I and II posted for elective laparoscopic cholecystectomy. Thoracic epidural anaesthesia was performed with 0.5% bupivacaine and fentanyl by the standard protocol. Various study parameters i.e. onset and duration of sensory and motor block, quality of sensory and motor block, hemodynamic responses, perioperative complication and postoperative analgesia were recorded and statistically analyzed. Results: The thoracic epidural anaesthesia was effective for laparoscopic cholecystectomy in all except in three patients where conversion to general anaesthesia was required. The hemodynamic parameters and respiratory efficiency were maintained within physiological limits, during pneumoperitoneum with minimal treatable side effects. Conclusion: The thoracic epidural anaesthesia successfully and effectively can be used for laparoscopic cholecystectomy avoiding airway manipulation, better postoperative analgesia and lesser hospital stay.
